We have reports of another senseless trenching fatality. This one occurred in California, but mimics many of the others we have seen in Ohio and throughout the U.S. In April, the contractor was dismantling a trench box while a worker was still inside. Another worker was using an excavator to remove 5,000-pound, shoring-system rails when a bridle sling hook failed, dropped a rail and crushed the worker inside the trench box. Cal/OSHA issued a total of nine citations — five general, two serious, one serious accident-related and one willful-serious accident-related — for a variety of violations, including failure to: ensure that no workers were in the trench shield while it was being dismantled; use adequate bridle-sling hooks; inspect the excavation site daily for potential hazards and train employees on working safely around excavations. “Shield systems are designed to protect employees from cave-ins when working in an excavation,” said Juliann Sum, Cal/OSHA chief. “Employers must ensure that no one is inside of the excavation when the protective system is being installed or removed.” Cal/OSHA cited the contractor and proposed the company pay a $141,075 fine. Do you think the family of the deceased employee feels any better because a fine was issued? Fines can be paid, and work goes on…. but a life once lost can never be restored. As cold weather sets in and operations grind to a halt, it is a good time to take stock of your excavation and trenching safety procedures. ODOT Plan Reading Course Wednesday, January 30, 2019
OCA’s Chris Engle will be instructing this plan reading course once again. Before joining OCA, Chris spent 22 years with ODOT, with several years overseeing Construction in District 5. Training for this informative course covers reading, understanding and using ODOT plans. Includes sections, profiles, quantity take-offs, and relationships between components of the plans. The course is designed for someone who is just getting familiar with how to read plans. Includes lecture discussions and in-class use of plans.
